Notification of pregnancy and issuance of the Maternal and Child Health Handbook
Since July 2017, maternal and child health handbooks have been issued by each ward health and welfare center.
The maternal and child health handbook, which was previously issued at medical institutions within the city, is now issued by midwives and other professionals at each ward's health and welfare center, where consultations before and after childbirth are available. Once you have been diagnosed with pregnancy, please register your pregnancy at the health and welfare center (health division) in your ward.
The Maternal and Child Health Handbook is an important document that records the health status of the mother and child, including the progress of pregnancy, the circumstances of birth, and the child's growth and development after birth.
Once you have been diagnosed with pregnancy at a medical institution, please register your pregnancy as soon as possible at your local health and welfare center (health department) and receive your maternal and child health handbook. You will also receive a booklet of vouchers for prenatal and postnatal health checkups.
*Since April 1, 2025, we have been providing a support benefit for pregnant women to alleviate the physical, mental, and economic burden on pregnant women during the prenatal and postnatal periods, and to contribute to the improvement of the health and welfare of pregnant women and their unborn children.

Notification of pregnancy (issuance of the Maternal and Child Health Handbook)
You will need to complete the procedure at the Health and Welfare Center in your ward.
Pregnancy notifications and issuance of maternal and child health handbooks are handled individually at the counter.
Please visit your local ward's Health and Welfare Center (Health Division) between 9:00 and 17:00 on business days.

Documents required when notifying pregnancy
When the pregnant woman applies for the procedure herself
- Pregnancy Notification Form (issued by medical institutions. If you are attending a medical institution outside the city, etc. and do not have a pregnancy notification form, please contact your local ward health and welfare center.)
- Documents verifying your personal number (My Number)
Documents verifying your individual number (individual number card, number notification card, certificate of residence showing your number, document issued by a government agency showing your number) - Identification of the pregnant woman
Individual number card, photo card, driver's license, passport, residence card, etc.
*Two of the following documents are required: Public health insurance/insured card, child support certificate, copy of residence certificate, etc.
When a family member or other representative is completing the procedure (when the person in question is unable to come to the office due to poor health, etc.)
- Pregnancy Notification Form (issued by medical institutions. If you are attending a medical institution outside the city, etc. and do not have a pregnancy notification form, please contact your local ward health and welfare center.)
- Pregnant woman's personal number confirmation document
Individual number card, number notification card, resident's card with number printed on it, etc. - Identification documents of the representative
Driver's license, passport, personal number card, etc.
*Due to amendments to the Maternal and Child Health Act, the pregnant woman's My Number (personal number) is now required on the Pregnancy Notification Form.
Please note that we will use the above documents to verify the pregnant woman's personal number and identity.
