Health check-up for 5-year-olds

We conduct health checkups for five-year-olds, a crucial period for motor skill development and psychological/emotional development, to support the healthy growth of your children.

We have sent out notifications to eligible individuals. If you have received a notification, please follow the instructions below.

While not all 5-year-olds are required to attend the group health checkup, please use the self-check results as a reference when scheduling your appointment.

Eligible persons

Children who are registered residents of Fukuoka City and will celebrate their 5th birthday during the 2026 fiscal year (born between April 2, 2021 and April 1, 2022)

Flow of consultation

1. We will send a "Notice Regarding 5-Year-Old Health Checkup" to eligible individuals at the time of their checkup.

2. Please answer the 5-year-old health check self-assessment by scanning the QR code provided in the notice.

　　If you are unable to access the 5-year-old health check self-check, please print out this self-check form and use it.

3. If you wish to have your 5-year-old child undergo a group health checkup based on your self-check results, please make a reservation.(Optional).
　　* The reservation page will be displayed after you have completed the self-check.
　　* Group health checkups are conducted at the health and welfare center in your ward.
　　　　The available dates for booking are listed on the booking page.
　　　　There are limited dates available for booking, so please apply as soon as possible.
　　*　Even if you wish to participate in a group health checkup, please understand that we may not be able to accommodate your request depending on the circumstances.

Date and time of group health checkup

The dates on which you can book a group health checkup are listed on the booking page that appears after you complete the self-check.
On the designated day (weekday), between 1:00 PM and 4:30 PM (approximate).(Registration is open from approximately 12:40 to 14:30)
Estimated time: 1-2 hours

*To receive a group health checkup,I have never consulted, visited, or received developmental therapy at a developmental therapy center before.This is for children.
If you have previously consulted with, visited, or received developmental support from a developmental support center, please contact the following:
○If you have any concerns about your child's health, please consult your family doctor.
○If you have any concerns about your child's condition or development, please contact the Fukuoka City Rehabilitation Center/AiAi Center where you previously received treatment.

Medical interviews, physical measurements, group play, pediatric examinations, childcare-related talks, etc.
*Vision tests, urine tests, and dental checkups are not included.

Requests for medical examinations

• If your child or accompanying person is unwell, please refrain from coming to the clinic on the scheduled day.
• To avoid overcrowding, please allow only one person to accompany each child into the health check-up venue.
• This is not a place to diagnose developmental disorders or similar conditions.
