How to take a health check

• You can have your baby examined at a pediatric or obstetrics and gynecology clinic in the city using the "One-Month-Old Baby Health Checkup Ticket."
• If you already have a maternal and child health handbook: You can pick it up at the medical institution you visit or at your ward health and welfare center.
  For those who will receive a maternal and child health handbook: It will be issued together with the maternal and child health handbook at the ward health and welfare center.

• If you visit a medical institution outside the city, such as when returning home, you may be eligible for a certain amount of subsidy from Fukuoka City.

If the results of the 3-year-old infant health check indicate that more detailed testing is necessary, a referral letter will be given.

Eligible persons

Children who are registered residents of Fukuoka City and will celebrate their 5th birthday during the 2026 fiscal year (born between April 2, 2021 and April 1, 2022)
